At Roots, aftercare planning starts on day one. Before discharge, we work with each family to prepare for the transition home, build a clear next-step plan, and support reintegration with care.
The goal is not just to discharge your daughter. It is to help her leave with structure, support, and a stronger path forward.
A plan for the next step
Before she leaves the ranch, our clinical team works with you to create a reintegration plan. This isn’t a generic packet of papers. It’s is a step-by-step guide for your specific family. This plan includes
- A discharge plan and reintegration plan designed for her next step
- Coordination with outpatient providers and aftercare resources
- Coaching to support family reintegration
- Continued case management through the discharge process
You are never alone in the process
The transition home can feel vulnerable for both teens and families. Roots stays connected through that process so you are not left to figure everything out on your own.
After graduation, girls can stay engaged through ongoing alumni support, shared resources, and continued connection to the Roots community.
- Weekly alumni groups: Your daughter can log in to see familiar faces and talk through her new challenges with peers who truly get it.
- The Roots app: A private, secure community where she can access tools, inspiration, and support 24/7.
- Case management: Our team stays involved to ensure the transition to local providers is smooth and successful.
